PENGARUH TERAPI ELECTRICAL MUSCLE STIMULATION (VEINOPLUS ARTERIAL) TERHADAP PROSES PENYEMBUHAN LUKA PADA PASIEN UKD DI KLINIK KITAMURA PONTIANAK
Abstract
Abstract: Diabetic Foot ulcer (UKD) is a chronic complication of Diabetes Mellitus with incidence rate of 15%, to increase the wound healing process one of the methods that can be done is to use Electrical Muscle Stimulation (EMS) therapy which is a procedure by producing short electric waves aimed to imitate the movement normal body and stimulates wound healing. The aim of this study was to identify the effect of EMS on calf muscle on wound healing in UKD patients. This research uses pure experimental design (true-experiment) with randomized controlled trials (RCT). The total sample involved 60 respondents divided into two groups respectively 30 respondents. Analysis of data by using Wilxocon Test analysis on mean comparison of Ankle Bracial Index (ABI) and Dependent T-test analysis on mean comparison of MUNGS score. Multivariate data analysis used was MANOVA with GLM-RM method on repeated measurement of dependent variable. EMS therapy on calf muscle using Veinoplus Arterial tool obtained P value 0.00 <0.05, which means there is significant difference between before and after treatment (control and intervention) to ABI value and MUNGS score. Multivariate test results were obtained by p value 0.00 <0.05, which means that there are significant differences between multiple measurements of ABI and MUNGS scores, but the plot graph shows no optimal time in EMS therapy. The researchers advise that EMS therapy can be applied as one of the management of UKD patients. Further research needs to tighten the inclusion criteria of respondents with ABI <0.9, and add time> 7 days to be able to assess the optimum time of EMS therapy.

Abstrak: insiden 15%, untuk mempercepat proses penyembuhan luka salah satu metode yang dapat dilakukan adalah menggunkan terapi Electrical Muscle Stimulation (EMS) yang merupakan prosedur dengan menghasilkan gelombang elektrik yang pendek bertujuan meniru pergerakan tubuh yang normal dan menstimulasi penyembuhan luka. Tujuan penelitian ini untuk mengidentifikasi efek dari EMS pada otot betis terhadap penyembuhan luka pada pasien UKD. Penelitian ini menggunakan desain Eksperimen murni (true-experiment) dengan randomised controlled trials (RCT). Total sampel yang terlibat 60 responden dibagi dalam dua kelompok masing-masing 30 responden. Analsis data dengan menggunakan analisis Wilxocon Test pada perbandingan rerata nilai Ankle Bracial Index (ABI) dan analisi T-tes Dependent pada perbandingan rerata Skor MUNGS. Analisis data multivariat yang digunakan adalah MANOVA dengan metode GLM-RM pada pengukuran berulang variabel dependen. Terapi EMS pada otot betis dengan menggunakan alat Veinoplus Arterial diperoleh hasil P value 0.00 < 0.05, yang artinya terdapat perbedaan yang signifikan antara sebelum dan sesudah perlakuan (kontrol dan intervensi) terhadap nilai ABI dan Skor MUNGS. Hasil uji multivariat di dapatkan p value 0.00 < 0.05, yang artinya terdapat perbedaan yang signifikan antara beberapa kali pengukuran terhadap nilai ABI dan Skor MUNGS, namun pada grafik plot menunjukkan belum ditemukan waktu yang optimal dalam pemberian terapi EMS. Peneliti memberikan saran agar terapi EMS dapat diterapkan sebagai salah satu penatalaksanaan pasien UKD. Penelitian lebih lanjut perlu memperketat kriteria inklusi responden dengan ABI < 0.9, serta menambahkan waktu > 7 hari untuk dapat menilai waktu optimum terapi EMS.
